Okay, what's the catch?
As all offers, this one comes with some fine print. However, it's pretty par for the course with not "gotcha" bits:
This deal requires a new line of service activation and an 18-month lease. Well-qualified customers are eligible.
There are taxes on any device you go with and you'll have to pay that up front regardless.
If you cancel your service, the remaining device balance will become due.
Do I have to choose a specific plan?
From everything we're reading, you don't have to pick a particular plan with Sprint to take advantage of this offer. However, they are promoting it with their Unlimited Freedom plan, which is $50 per month for unlimited data, but if you want to add extra lines, each one is only $22.50 after the first (up to 4 lines, and then the 5th one is actually free).
This might be worth looking into, because the Unlimited Freedom plan is currently going at a lower introductory price, after which they're anticipating moving to a $60 per month price structure.
